Job Summary
We are seeking a dedicated and compassionate Case Manager - Qualified Mental Health Professional to join our dynamic healthcare team. We have different locations in Virgina. In this vital role, you will coordinate comprehensive mental health services for patients across inpatient, outpatient, and post-acute care settings. Your expertise will help facilitate patient recovery, ensure seamless discharge planning, and promote optimal health outcomes. Responsibilities
- Conduct thorough patient assessments to evaluate mental health status, medical history, and social needs, utilizing your knowledge of physiology and medical terminology.
- Develop individualized care plans in collaboration with multidisciplinary teams, including physicians, social workers, and addiction counselors.
- Coordinate inpatient and outpatient services, ensuring timely access to appropriate behavioral health resources and community support programs.
- Manage discharge planning processes by arranging post-acute care, outpatient therapy, or specialized clinics such as PICU or Level I Trauma Center units.
- Perform diagnostic evaluations and monitor patient progress through ongoing case management activities.
- Maintain accurate and confidential medical records in compliance with HIPAA regulations and hospital policies.
- Engage in utilization management by reviewing medical records and supporting diagnostic evaluations to optimize patient care pathways.
- Facilitate intake procedures for new patients and assist with medical records documentation to ensure seamless service delivery.
Requirements
- Valid licensure as a Qualified Mental Health Professional.
- Experience in behavioral health, mental health counseling, addiction counseling, or related fields with a focus on pediatrics or managed care.
- Knowledge of medical terminology, physiology, patient assessment techniques, and discharge planning processes.
- Familiarity with HIPAA regulations and medical records management to ensure patient confidentiality and compliance.
- Excellent communication skills for effective collaboration with healthcare teams and patients alike.

Pay: $60.00 - $85.00 per hour
