Case Manager-Recovery House

Lake Area Recovery Center has an immediate, full-time position available for a Recovery House Case Manager.

Responsibilities include:

• Provide ongoing support and case management expertise to clients diagnosed with Substance Abuse disorders
• Work with clients to develop a case management plan
• Ensure that all case management plans address identified needs and are up to date
• Continually monitor and evaluate each client’s progression through their case management plan and develop corrective action revisions to the plan as needed
• Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Lake Area Recovery Center guidelines
• Prepare case management related reports including but not limited to weekly Drug Court reports
• Connecting individuals to treatment options including but not limited to mental health services and physical health
• Assisting with application processes such as Medical Assistance, Social Security, job applications and educational opportunities
• Advocating for individuals to probation, courts, housing entities etc...
• Reducing barriers to services by linking to available resources such as transportation and childcare
• Coaching individuals on life skills which includes resume building, household management, budgeting, etc.
• Connecting individuals to basic needs such as food and clothing
• Adheres to strict boundaries and professional ethics
• Attend staff meetings on a weekly basis
• Previous experience as a Case Manager preferred
• CDCA preferred
