Case Manager Residential Substance Abuse

Thoroughbred Wellness and Recovery is a dedicated substance use treatment provider offering detox and residential services to adults. We are seeking a compassionate and highly organized Case Manager to join our team. As a Case Manager, you will play a pivotal role in coordinating and delivering personalized care and support services to our clients, ensuring they achieve their goals and improve their quality of life. Your responsibilities will involve assessing client needs, developing care plans, facilitating therapeutic groups and collaborating with a multidisciplinary team to deliver comprehensive support.

This is a Tuesday through Saturday position.

JOB SUMMARY

A Case Manager plays a pivotal role in coordinating and delivering personalized care and support services to our clients, ensuring they achieve their goals and improve their quality of life. Responsibilities involve assessing client needs, developing care plans, and collaborating with a multidisciplinary team to deliver comprehensive support.

KEY RESPONSIBILITIES:

Client Assessment: Conduct biopsychosocial assessments upon admission by collecting and reviewing medical, mental health, and substance use histories. Coordinate with clinical and medical teams to determine appropriate levels of care.

Care Coordination: Proficiency in researching various conditions, treatments, and services, coupled with the ability to create comprehensive, personalized, and evidence-based care plans that address the unique needs of each client. Collaborate and coordinate with clinicians to develop treatment plans based on these initial assessments. Advocate for appropriate care and streamline the administrative tasks associated with accessing services. Coordinate referrals, transportation, applications, and social resources.

Group Therapy: Facilitate group therapy sessions focused on substance use recovery, mental health management, relapse prevention, and other related topics.

Documentation: Maintain accurate and up-to-date records of client interactions, assessments, care plans, and progress notes in accordance with company policies and regulatory requirements.

Collaboration & Team Support: Work closely with therapists, counselors, nurses, and medical staff to support individualized treatment plans. Participate in staff meetings, training, and ongoing education related to mental health, substance use, and trauma-informed care. Uphold confidentiality, ethical standards, and regulatory compliance at all times.

QUALIFICATIONS

Education: Bachelor’s or associate degree in social work, psychology, counseling, or a related field preferred. Minimum is a high school diploma or GED.

Licensing & Certifications: At minimum, certification in substance use disorder treatment (e.g., CADC, CAC) in the state of Georgia. CPR and de escalation required (provided by facility).

Experience: Prior experience in substance abuse treatment, detox, residential.

  • Strong communication and organizational skills
  • Cultural competence and trauma-informed care understanding
  • Ability to handle crisis situations calmly and ethically
  • Knowledge of local resources and recovery support systems
