Case Manager (RN or Social Worker)

Job Summary
The Case Manager is responsible for overseeing residents in both long-term care and short-term settings. The role focuses on compliance, discharge planning, advocating for residents, and provide leadership and supervision of the social services department within the facility. The position will be supporting residents and families with admissions, care planning, transition of care, and long-term placement and discharging to home or other facilities

Duties & Responsibilities

Leadership & Oversight

  • Direct and manage the Social Services Department for long-term care and rehabilitation units.
  • Ensure compliance with federal, state, and local regulations
  • Develop successful discharge planes for residents.
  • Provide supervision, guidance, to or social services

Case Management & Resident Support

  • MDS completion and other required documentation
  • Develop, update individualized care plans.
  • Facilitate interdisciplinary care plan meetings.
  • Provide counseling, and emotional support to residents and families.
  • Address concerns related to grief, behavioral health, family conflict, and resident rights.
  • Coordinate referrals, community resources and outside agencies.

Discharge Planning

  • Coordinate timely discharges for rehab residents
  • Arrange home health services, DME, transportation, follow-up appointments, and community supports.
  • Communicate discharge plans with families, residents and team.
  • Monitor readmission risks and implement preventative interventions.

Regulatory & Compliance

  • Documentation meets state regulatory requirements.
  • Maintain accurate charting.
  • Protect resident confidentiality
  • Advocate for resident rights

Collaboration

  • Works closely with Nursing, Therapy, Admissions, Billing, Activities, and Dietary departments.
  • Is liaison between residents, families, and team.
  • Part of the Quality Assurance and Performance Improvement (QAPI) team.
  • Ensure compliance with federal, state, and local regulations
  • Develop successful discharge planes for residents.
  • Provide supervision, guidance, to or social services

<Qualifications>

  • Bachelor’s Degree in Social Work (BSW) or Registered Nurse RN.

Licensure:

  • Current state Social Work license (LSW) or Nursing License (RN)

Experience:

  • Minimum 2 years of experience in long-term care, skilled nursing, or rehabilitation setting preferred.
  • Prior supervisory or leadership experience required.

Knowledge, Skills, & Abilities

  • Strong knowledge of long-term care regulations and discharge planning requirements.
  • Understanding of Medicare, Medicaid, and insurance processes.
  • Excellent communication and conflict resolution skills.
  • Able to manage several cases in a fast-paced environment.
  • Strong documentation and organizational skills.
  • Compassionate, resident-centered approach.
