Marywood is seeking qualified candidates for a Full-Time Case Manager (RN/LPN) position.
Essential Functions include, but are not limited to:
The Case Manager plays an integral role in optimizing patients’ recovery journeys. The Case Manager completes chart reviews and communicates with the inter-disciplinary care team to align expectations of residents’ length of stay. By serving as the link between patients and the appropriate health care personnel, the Case Manager is responsible for ensuring efficient, smooth, and prompt transitions of care.
Responsibilities include, yet are not limited to:
- Perform Skilled Nursing Facility (SNF) clinical chart reviews on patients using clinical skills, PDPM, and CMS criteria, upon admission to SNF and periodically through the patient stays
- Review targets for Length of Stay (LOS), target outcomes, and discharge plans with Health Care Plan
- Complete all SNF concurrent reviews, updating Health Care Plans in a timely manner.
- Attend Medicare Meetings to ensure accurate, current knowledge of resident conditions and estimated LOS
- Attend Morning Meeting
- Collaborate effectively with the patients’ health care teams to establish an optimal discharge. The health care team includes physicians, referral coordinators, discharge planners, social workers, physical therapists, etc.
- Assure appropriate referrals are made to the Health Plan for prior authorizations prior to admission and reauthorizations on a weekly basis. Review referrals based on PDPM reimbursement and CMS guidelines to determine appropriateness of accepting resident to facility
- Assess and monitor patients’ continued appropriateness for SNF setting (as indicated)
- Review referral requests that have not been approved for continued stay and communicate with the social worker/discharge planner for issuance of the NOMNC when appropriate
- Manage assigned caseload in an efficient and effective manner utilizing time management skills
- Enter timely and accurate documentation into the Health Plan review system
- Daily review of census and identification of barriers to manage independent workload and ability to assist others
- Adhere to organizational and departmental policies and procedures
- Maintain confidentiality of all PHI information in compliance with HIPPA, federal and state regulations, and laws
- Assist as MDS Nurse per facility need, at the discretion of the MDS Director
- Perform other duties and responsibilities as required, assigned, or requested
Qualifications:
- Active, unrestricted RN or LPN clinical license in the State of Michigan required
- 3 - 5 years of clinical experience required
- At least 2 years of Case Management/MDS experience preferred
- Proficient with Patient Driven Payment Model (PDPM) and NTA knowledge
- ICD10 Coding Basic Knowledge
- Experience working with geriatric population preferred
- Exceptional verbal and written interpersonal and communication skills
- Strong problem solving, conflict resolution, and negotiating skills
- Proficiency with Point Click Care preferred
- Must be familiar with insurance and provider electronic portals
- Independent problem identification/resolution and decision-making skills
- Detail oriented
- Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously
- Team player
- Must be able to move intermittently throughout the day
- Must be able to speak and write the English language I an understandable manner
- Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
- Must meet general health requirements set forth by the policies of the facility, which include a medical and physical examination
- Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet
- May be necessary to assist in the evacuation of residents during emergency situations

Pay: $34.00 - $38.00 per hour
