Case Manager, Specialty Behavioral Health

Summary of Position:

The Case manager Specialty Behavioral Health provides high quality patient-centered care coordination as an integral part of a dynamic multi-interdisciplinary team. Care is provided in the clinic and outside in the community meeting each of our patients “where they are”.

Essential duties:

Duties include, but are not limited to:

  • Attends team huddles when in clinic, clinic staff meetings, and team meetings.
  • Conducts patient consultations, assessments, and care planning in collaboration with healthcare professionals.
  • Educates patients on conditions, treatment options, and available resources.
  • Coordinates services, referrals, and transitions of care, including housing, employment, and crisis interventions.
  • Monitors patient progress, responding to changing needs, and facilitating re-engagement using trauma-informed approaches.
  • Advocates for patients and ensures compliance with healthcare regulations and confidentiality standards.
  • Supports hospital discharge planning and follow-up to reduce readmissions.
  • Completes documentation in the electronic medical record and manages prior authorizations.
  • Serves as a point of contact for patients and advocates to resolve service-related issues.
  • Performing other duties as assigned

Qualifications and Education:

Basic Knowledge and Skills:

  • High school diploma or GED work experience.
  • Required to maintain a valid Arizona driver’s license and pass motor vehicle screening.
  • BLS Certification
  • Fingerprint clearance issued by Arizona Department of Public Safety required.
  • Experience in working with underserved populations is highly desirable.
  • Must be at least 21 years of age.

Experience:

  • Minimum of 4 years’ experience of full time behavioral/integrated healthcare experience.US

Skills:

  • Organization and care coordination
  • Communication and teamwork
  • Proficient with HMIS.
  • Crisis intervention and de-escalation skills
  • Advocacy for client needs and service access
  • Knowledge of community resources and systems navigation, employment resources and Vocational Rehabilitation referral processes.
  • Knowledge of substance use and/or mental health disorders
  • Trauma-informed care
