Summary of Position:
The Case manager Specialty Behavioral Health provides high quality patient-centered care coordination as an integral part of a dynamic multi-interdisciplinary team. Care is provided in the clinic and outside in the community meeting each of our patients “where they are”.
Essential duties:
Duties include, but are not limited to:
- Attends team huddles when in clinic, clinic staff meetings, and team meetings.
- Conducts patient consultations, assessments, and care planning in collaboration with healthcare professionals.
- Educates patients on conditions, treatment options, and available resources.
- Coordinates services, referrals, and transitions of care, including housing, employment, and crisis interventions.
- Monitors patient progress, responding to changing needs, and facilitating re-engagement using trauma-informed approaches.
- Advocates for patients and ensures compliance with healthcare regulations and confidentiality standards.
- Supports hospital discharge planning and follow-up to reduce readmissions.
- Completes documentation in the electronic medical record and manages prior authorizations.
- Serves as a point of contact for patients and advocates to resolve service-related issues.
- Performing other duties as assigned
Qualifications and Education:
Basic Knowledge and Skills:
- High school diploma or GED work experience.
- Required to maintain a valid Arizona driver’s license and pass motor vehicle screening.
- BLS Certification
- Fingerprint clearance issued by Arizona Department of Public Safety required.
- Experience in working with underserved populations is highly desirable.
- Must be at least 21 years of age.
Experience:
- Minimum of 4 years’ experience of full time behavioral/integrated healthcare experience.US

- Knowledge of community resources and systems navigation, employment resources and Vocational Rehabilitation referral processes.
