Case Manager- Substance Abuse

Overview – Case Manager

We are seeking a dedicated and compassionate Behavioral Health Case Manager to join our team, specializing in Substance Use Disorder (SUD) services. This role is essential in supporting clients throughout treatment, promoting housing stability, and ensuring consistent engagement in programming. The ideal candidate will have experience in behavioral health and demonstrate a strong commitment to recovery-oriented, client-centered care.

Key Responsibilities

  • Participate in weekly multidisciplinary team meetings to provide case updates and collaborate on client care.
  • Ensure all case management activities align with and support individualized treatment goals.
  • Complete all documentation within 24 hours in accordance with agency, state, and regulatory standards.
  • Oversee and coordinate Safe and Sober housing.
  • Monitor client attendance in treatment programming and address absences in a timely manner.
  • Assist clients with transportation, access to community resources, and recovery supports to minimize disruptions in care.
  • Identify barriers to treatment engagement and escalate concerns that may impact adherence or outcomes.Schedule follow-up appointments prior to discharge and provide warm handoffs to ensure continuity of care.
  • Reinforce safety plans during crises and connect clients with appropriate emergency resources.
  • Accurately document all crisis-related activities to support quality assurance and compliance.
  • Facilitate group sessions as assigned, focusing on skill-building, recovery education, and client engagement.
  • Participate in a shared, periodic on-call rotation to provide after-hours support for urgent client or housing-related needs.

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
  • At least one (1) year of experience in substance use disorder treatment or behavioral health services.
