POSITION PURPOSE:
The Case Manager is responsible for overseeing the overall wellness of the clients served at Arrowwood Addiction Treatment Center to ensure they are supported and can achieve the best outcome.
POSITION REQUIREMENTS/QUALIFICATIONS
1. EDUCATION:
Graduated from an accredited college or university with a minimum of a Bachelor’s Degree is required. Degree in Psychology, Social Work, Criminal Justice or health-related field preferred.
2. EXPERIENCE:
A minimum of 2 years of case management experience working directly with clients having substance related disorders and 2 years of group therapy experience using a variety of treatment modalities preferred.

- Knowledge of community resources with the ability to engage resources and achieve coordination of care for a client.
- Ability to advocate for the client with a variety of service providers.
- Demonstrate effective communication skills both oral and written.
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
- Responsible for completing all intake paperwork and documenting in the client’s EMR.
- Responsible for providing on-going supportive and/or case management functions in accordance with the problems, needs, preferences of the client to assist the client in achieving long-term sustained sobriety.
- Responsible for developing and maintaining a community resources catalog for continuing care services.
- Responsible for developing an aftercare plan with the client at admission and providing ongoing face to face reviews with the client on a regular basis to assess the progress made in moving towards successful completion of the program.
- Responsible for developing the discharge plan and coordination of aftercare services.
- Responsible for documenting all care coordination contacts on the same day services are rendered in the clients EMR, including but not limited to: face-to-face, collateral and outside resources.
- Responsible to participate in clinical treatment team meetings, as scheduled.
- Responsible for participating in regularly scheduled supervision as a means of enhancing professional growth, reviewing the provision of clinical services and addressing administrative issues.
- Responsible for maintaining, as required, collaborative relationships with referring agencies to formulate appropriate collaborative intervention strategies.
- Responsible to conduct waived testing according to Drug Testing policy and procedures.
- Responsible for the completion of incident reports, as necessary.
- Performs other duties as assigned.
Job Type: Full-time
Pay: $45,000.00 - $55,000.00 per year
