Job Title: Case Manager / Therapist

Position Overview
The Case Manager / Therapist at FANM plays a vital role in delivering trauma-informed, culturally competent social services to low- to moderate-income families, immigrants, and underserved populations throughout South Florida. This position combines comprehensive case management with therapeutic support services, including individual client support, psychoeducation, crisis intervention, and facilitation of support groups in Haitian Creole and English.
The ideal candidate will have strong experience in social services and therapeutic practice, with the ability to address complex family, emotional, behavioral, and systemic challenges. This role requires fluency in both English and Haitian Creole, as the Case Manager / Therapist will regularly conduct assessments, provide direct services, and lead workshops or support groups for Haitian Creole speaking clients.
This position is essential in helping clients achieve self-sufficiency, emotional wellness, family stability, and empowerment through advocacy, direct support, and coordinated care.
Key ResponsibilitiesClient Intake, Assessment & Therapeutic Support
- Conduct comprehensive biopsychosocial assessments and client intakes to identify immediate needs, barriers, strengths, and goals.
- Assess mental health, trauma, family dynamics, housing instability, and social service needs.
- Provide supportive counseling, crisis intervention, and therapeutic guidance to individuals and families.
- Develop individualized service and wellness plans that integrate case management and therapeutic objectives.
- Maintain confidential, accurate, and timely case documentation in compliance with agency standards.
Case Management & Service Coordination
- Connect clients with internal FANM programs and external resources, including:
- Housing assistance
- Immigration/legal support
- Public benefits
- Employment services
- Mental health resources
- Parenting support
- Healthcare access
- Coordinate care with schools, healthcare providers, legal agencies, shelters, and community organizations.
- Advocate for clients navigating social systems, public services, and community resources.
- Monitor and evaluate client progress, adjusting plans as needed.
Group Facilitation & Community Education
- Facilitate psychoeducational groups, parenting classes, wellness workshops, and support groups in Haitian Creole and English.
- Lead culturally responsive therapeutic group sessions addressing:
- Parenting support
- Family conflict
- Emotional wellness
- Immigration-related stress
- Housing instability
- Trauma recovery
- Create safe, supportive environments that promote healing, resilience, and empowerment.
Advocacy & Community Engagement
- Advocate on behalf of clients with schools, landlords, healthcare providers, and government agencies.
- Support community outreach initiatives targeting Haitian, immigrant, refugee, and underserved communities.
- Participate in community events, workshops, and partnership-building activities.
Documentation, Reporting & Compliance
- Complete case notes, assessments, progress reports, and service outcomes accurately and on time.
- Assist with grant reporting and program evaluation requirements.
- Participate in supervision, case reviews, and professional development trainings.
- Ensure adherence to ethical, legal, and organizational standards.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, Sociology, Human Services, or related field required.
- Master’s degree in Social Work, Counseling, Marriage & Family Therapy, or related clinical field preferred.
- Minimum of 2 years of experience in:
- Case management
- Social services
- Behavioral health
- Counseling or therapeutic services
- Demonstrated experience providing therapeutic support, counseling, or mental health interventions.
- Experience facilitating support groups, parenting workshops, or psychoeducational groups.
- Fluent in both English and Haitian Creole (written and verbal) — REQUIRED.
- Strong understanding of immigrant, refugee, Haitian, and underserved community needs.
- Knowledge of trauma-informed care, crisis intervention, and culturally responsive service models.
- Excellent communication, documentation, and organizational skills.
- Ability to maintain confidentiality and professional ethical standards.
Preferred Qualifications
- Licensed or license-eligible social worker, therapist, or counselor (LCSW, LMHC, LMFT, etc.)
- Experience with family systems, child welfare, or community mental health
- Familiarity with Miami-Dade, Broward, and Palm Beach social service networks
- Experience with public benefits, immigration systems, and housing advocacy
Work Environment
- Primarily office-based with hybrid flexibility when appropriate
- Regular direct service with clients in office, community settings, and virtual platforms
- Occasional travel within Miami-Dade, Broward, and Palm Beach Counties
