Position: Case Manager, Women’s Residential Treatment
Location: Fayette, OH
Classification: Full Time, Hourly, Non-Exempt
The Case Manager provides case management services, as defined by CARF and OMHAS. In addition, the Case Manager maintains primary responsibility for clients assigned to her caseload/group and coordinates treatment activities with staff and consultants as appropriate. The Case Manager reports to her Direct Supervisor.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

Assist clients in scheduling appointments including medical, legal, Human Services, Social Security, etc.

Assist the clients who can work in obtaining employment in the area, and assist client in seeking new employment opportunities.

Maintain up to date client records with all appropriate documentation, including billing documentation in accordance with agency policies and procedures.

Perform drug testing and documentation as assigned.

Assist in discharge planning, including linkage to other services (housing, outpatient treatment, etc.)

Symptom monitoring and coordination with physician and nurse.

Coordination and/or assistance in crisis management and stabilization as needed.

Assists clients in understanding their mental illness and chemical dependency issues.

Assist with accessing transportation. Supervise and assist in the linking of clients to and from appointments when necessary.

Functions as primary and/or co-counselor in individual and group counseling sessions as license allows.

Develops an assessment, ISP review, continued care plan, and discharge summary plan for each client on her caseload as license allows.

Responsible for upkeep on vehicle assigned including scheduling maintenance, keeping mileage logs, and making sure safety equipment is updated and maintained.
MINIMUM QUALIFICATIONS:

High School diploma required, Associates Degree in Social Work or helping related field or Bachelor Degree in Social Work preferred.
- Chemical Dependency Counselor Assistant (CDCA) license required. Licensed Social Worker preferred.

Training and experience in the mental health and addiction field required.

Valid driver’s license, proof of insurance, and ability to be insured by agency provider.

Must be 21 years of age.

Subject to BCI background check, pre-employment drug screen, and reference checks.
