Case Manager working with Adults Hope Housing

Eastway Behavioral Healthcare is the largest, private, not-for-profit provider of mental health, substance use disorder, housing, residential, and employment services in the Miami Valley. Housing Opportunities for People Everywhere (HOPE) is an Eastway program that works with adults with behavioral health and/or substance abuse disorders experiencing homeless. We are hiring a Community Support Specialist (CSS)/Case Manager for HOPE.

This position will participate in an interdisciplinary team of psychiatrist, nurse, therapist, vocational specialist, housing coordinator, and other community support specialists in support of the recovery and housing stability of persons with co-occurring disorders including serious mental illness and drug and alcohol dependence.

Duties

· Works as part of the HOPE Housing ACT Team of 10 employees.

  • Works as a team with all clients in the HOPE Housing program to help them achieve their goals and stable housing.
  • Actively works with clients who have limited awareness of illness and poor adherence to treatment regimes. Works to identify the underlying barriers to treatment adherence, i.e., lack of transportation, loss of entitlements, and lack of understanding of medications.
  • Educates family members about mental illness and actively works with family members to resolve issues effecting families, and utilizes family members as a resource in achieving positive outcomes with the client.
  • Participate in outreach efforts towards homeless clients in the shelter and in the community in order to build rapport and engage the client in appropriate services.
  • Strongly promotes the benefits of work in the process of recovery.
  • Develops Individual Recovery Plans which include assertive interventions that directly target the improvement of the client's activities of daily living and address the recovery support services involved in mental health and alcohol and drug treatment.

Qualification

· Bachelor’s Degree in Social Work, Psychology, Mental Health, or a related field OR at least 3 years of experience working in the mental health field.

· Chemical Dependency Counselor Assistant (CDCA) licensure OR prior experience working with individuals with addictions preferred.

· Prior experience working with the homeless.

· Valid Ohio Driver’s License with proof of car insurance required as this position requires driving clients.

· Experienced in Windows-based computer applications

Full-time Monday through Friday

Must be 21 yrs of age

Job Type: Full-time

Pay: $18.50 - $20.00 per hour
