Job Overview
We are seeking a dedicated and compassionate Case Manager/CBRS (Community-Based Rehabilitation Services) Worker to join our dynamic healthcare team. Duties
- Conduct detailed patient assessments, including medical history review, physiological evaluation, and behavioral health screening to determine individual needs.
- Develop and implement tailored care plans that incorporate discharge planning, utilization management, and post-acute care coordination.
- Collaborate closely with hospital teams such as social workers, physicians, nurses, and addiction counselors to ensure comprehensive patient support.
- Manage medical records and documentation accurately while maintaining strict adherence to HIPAA regulations to protect patient privacy.
- Facilitate intake processes for new patients, ensuring timely access to appropriate services within inpatient, outpatient, or clinic settings.
- Conduct diagnostic evaluations and coordinate with multidisciplinary teams for optimal patient care across various specialties including ICU, PICU (Pediatric Intensive Care Unit), trauma centers, and behavioral health units.
- Oversee utilization management activities by reviewing medical necessity and supporting appropriate resource allocation for inpatient and outpatient services.
- Assist with discharge planning by coordinating follow-up care, outpatient services, or community resources to promote recovery and prevent readmission.
Skills
- Extensive hospital experience with familiarity in Level I Trauma Centers and Level II trauma center environments.
- Proven expertise in acute care settings such as ICU, emergency medicine, OR (Operating Room), and pediatrics including PICU.
- Strong knowledge of medical terminology, physiology, mental health counseling, addiction counseling, and behavioral health principles.
- Skilled in patient assessment techniques with the ability to evaluate complex cases involving post-acute care or outpatient clinics.
- Experience managing medical records accurately while ensuring compliance with HIPAA standards.
- Proficiency in discharge planning processes along with utilization management and diagnostic evaluation skills.
