Case Manager/Community Based Rehabilitation Specialist (Boise)

Job Summary
Join our dedicated team as a Case Manager/Community Based Rehabilitation Specialist in Boise, where you will play a vital role in empowering individuals with developmental disabilities and autism to live fulfilling, independent lives. This energetic position involves coordinating personalized care plans, providing direct support, and fostering community integration. You will work closely with clients, families, and community partners to ensure comprehensive, compassionate services that promote growth, safety, and well-being. Your enthusiasm and proactive approach will help create positive outcomes for those we serve. Duties

  • Develop and implement individualized care plans tailored to each client’s unique needs and goals
  • Must be able to pass a background check
  • Facilitate community integration activities such as social outings, educational programs, and employment support
  • Collect and maintain accurate data on client progress, service delivery, and incident reports for compliance and quality assurance

Requirements

  • Must be 18 or older and have a Bachelor's Degree in behavioral science or other health and human services field
  • Knowledge of special education principles and behavioral health practices
  • Strong communication skills; ability to adapt messaging for diverse needs including using American Sign Language if applicable

Pay: $24.65 - $29.68 per hour
