Case Manager/Community Based Rehabilitation Specialist (Boise)

Job Summary
Join our dedicated team as a Case Manager/Community Based Rehabilitation Specialist in Boise, where you will play a vital role in empowering individuals with developmental disabilities and autism to live fulfilling, independent lives. • Develop and implement individualized care plans tailored to each client’s unique needs and goals
• Must be able to pass a background check
• Facilitate community integration activities such as social outings, educational programs, and employment support
• Collect and maintain accurate data on client progress, service delivery, and incident reports for compliance and quality assurance

Requirements

• Must be 18 or older and have a Bachelor's Degree in behavioral science or other health and human services field
• Knowledge of special education principles and behavioral health practices
• Strong communication skills; ability to adapt messaging for diverse needs including using American Sign Language if applicable
