Case Manager/Community Support Facilitator

We are growing our teams and are excited to welcome two new Case Managers / Community Support Facilitators who shares our passion for dignity, compassion, and hope.
About the Role
As a Case Manager / Community Support Facilitator, you’ll become a trusted guide and mentor for 8–10 clients living independently in their own homes. This role is about much more than just “support” it’s about helping people thrive. With care, creativity, and dedication, you will:
- Teach and encourage independence in everyday living skills like maintaining their home.
- Promote health and well-being through exercise, nutrition, and healthy lifestyle habits.
- Provide medication oversight and support clients in navigating medical appointments with confidence.
- Empower financial independence through everyday budgeting and money management guidance.
- Create opportunities for community involvement, friendship, and the development of social skills.
- Build strong, respectful partnerships with third-party agencies and community partners.
- Participate in collaborative team meetings to celebrate successes and plan for continued growth.
- Offer steady encouragement and guidance through life transitions and personal milestones.

Qualifications & Requirements
- Education: Bachelor’s degree in social work, psychology, special education, sociology, or related field preferred (not required).
- Experience: Previous work with adults with developmental disabilities preferred.
- Transportation: Reliable transportation, valid California Driver’s License, proof of minimum required auto insurance, and ability to be insured on company policy.
- Must pass a background check, pre-employment physical, and drug screening.
Compensation & Benefits
- Pay: $21.00 - $23.00 per hour depending on experience
- Job Type: Full-Time
- Schedule: Monday–Friday, 8-hour shifts
