NFI Massachusetts Inc., a nonprofit human service agency, is currently seeking a Case Manager/Family Support Worker for its Foundations for Families program located in the North Shore. Candidate must have lived experience as a parent/caregiver of a youth with significant social, emotional, behavioral health, medical or trauma related needs and experienced in navigating child and family serving systems.

NFI’s Foundations for Families program provides home based treatment, community based outreach, comprehensive assessment, counseling, case coordination, and crisis intervention services for youths and families within the Department of Mental Health, Department of Children and Families and Mass Health systems. Services and service intensity are highly individualized to meet the unique and varying needs of each youth/family.

Foundations for Families staff are highly trained and experienced in family engagement, child and adolescent development, serious emotional/behavioral/mental health issues, crisis management and de-escalation skills. Staffing patterns depend on the service the family is referred for and can include: licensed clinicians, masters level clinicians, parent partners who have life experience as parents of children with special needs and bachelor level Therapeutic Supports.

Services are available both on a planned, scheduled basis as well as a response to a youth/family in crisis. All services are flexible and swift to accommodate a family’s challenging needs. Services are directed toward and maximizing the youth/family’s strengths and keeping the family together, avoiding the need for out-of-home placement. The team will also ensure that families are provided with information and referral to community resources and supports to maximize the youth/family’s support network.

Foundations for Families services can operate as a “stand alone” service or it can be effectively integrated into larger service plans involving other services and providers such as hospitals, state agencies, outpatient services, residential treatment programs and other community-based programs.

Salary: $21-$24 per hour

Job Responsibilities:

  • Conduct intakes for new referrals to the program.
  • Educates and supports caregivers in navigating systems of care and how to access appropriate care for their child/grandchild/etc.
  • Assist and support caregivers in problem solving and advocacy skills.
  • Assist in locating and accessing resources.
  • Support caregiver in development strategies for self-care, identifying respite care providers and accessing respite care.
  • Provide in the moment modeling and coaching to increase parenting skills and sense of competency.
  • With guidance from youth, family, caregivers and other supports creates a strength based client focused assessment and treatment planning.
  • Provide individual and family therapeutic supports as defined by client’s service plan.
  • Work collaboratively with the clinician and/or other members of the treatment team.
  • Facilitates and/or participates in case conferences and treatment meetings as appropriate.
  • Writes necessary progress notes, aftercare plans, service plans, and reviews as defined by the program contract.
  • Transports clients as needed to court, appointments and home visits.
  • Assist other staff in crisis prevention and crisis management.
  • Perform all duties in accordance with the agency’s policies and procedures.

Job Requirements:

  • Must have lived experience as a parent/caregiver of a youth with a significant social, emotional, behavioral health, medical or trauma related needs and experienced in navigating child and family serving systems.
  • Bachelors preferred
  • Flexible schedule including evening and weekend availability.
  • Excellent interpersonal, organizational and writing skills required.
  • Valid driver’s license as travel is required.
