Case Manager/Group Facilitator

Join our dynamic behavioral healthcare team as a Case Manager/Group Facilitator, where you will play a vital role in coordinating patient care and facilitating group sessions to support recovery and well-being. Responsibilities

  • Conduct thorough patient assessments, including medical history review, physiological evaluation, and mental health screening, to determine individualized care needs.
  • Experienced in writing effective and professional notes.
  • Develop and implement tailored discharge planning strategies that ensure seamless transitions from inpatient to outpatient or post-acute care settings.
  • Facilitate engaging group sessions focused on behavioral health, addiction counseling, mental health support, life skills training groups and recovery education, promoting active participation and peer support.
  • Collaborate closely with multidisciplinary teams—including social workers, physicians, nurses, and therapists—to coordinate comprehensive patient care plans aligned with hospital protocols and managed care standards.
  • Manage medical records accurately while ensuring compliance with HIPAA regulations; document all case management activities thoroughly.
  • Assist with intake procedures for new patients, including diagnostic evaluations and utilization management to optimize resource allocation.
  • Provide education on medical terminology, physiology, and treatment options to patients and their families to empower informed decision-making.

Skills

  • Strong background in behavioral health. Lived experience is a +++++
  • Knowledge of outpatient clinic operations, post-acute care processes, and managed care principles.
  • Familiarity with medical terminology, diagnostic evaluation techniques, discharge planning procedures, and medical records management.
  • Excellent patient assessment skills combined with the ability to facilitate group discussions effectively
  • Understanding of HIPAA compliance standards and experience handling sensitive patient information securely.
