Case Management - Mental Health and Wellness Services

Nonprofit Human Services Organization is seeking the following:

Case Manager/Mental Health Rehab Specialist (Full Time)

To work with adult clients living in Dorchester County, Maryland providing mental health support services in group and individual settings.

Bachelor’s degree preferred, Associate's degree with relevant experience required.

Hours are generally Monday - Friday, 8a-4p or 8:30a-4:30p (depending on program needs, however other hours may be necessary at times)

We are looking for someone who is:

- Team oriented, flexible, energetic, and has a commitment to working with clients

- Willing to meet with clients both individually and through leading groups both on property and in the community or home settings

- Able to complete required documentation including service notes, assessments, treatment plans, etc. through an Electronic Health Record

- Able to provide transportation in company vehicles to clients for service delivery

- Able to participates in on-call rotation 3-4 times per year

- Maintains a valid driver's license with minimal (2) or no points on driving record, and has evidence of auto liability insurance

- Background check required and maintains a clear criminal background profile

- Willing to undergo drug testing. Channel Marker is a drug-free workplace.
