Case Manager/Program Coordinator

Independent Living Partners is seeking a highly organized and proactive Case Manager–Program Coordinator to serve as a central support resource for Waiver Case Management program. This position plays a key role in providing expert administrative and operational support to Program Managers, Case Managers, and the administrative team.

The ideal candidate is detail-oriented, dependable, punctual, and skilled at researching and resolving complex questions while working across multiple systems and teams.

Primary Responsibilities

  • Pull Support Plans from MnChoices 2.0, then print and mail them to clients and providers
  • Assign and update Case Managers to clients as needed
  • Locate and upload MnChoices assessment documents to cloud storage platforms (Pavillio)
  • Assign clients in SSIS to the appropriate Case Manager for transfers and new intakes
  • Serve as a subject matter resource across various programs
  • Attend county meetings, trainings, and informational sessions to stay current on program updates
  • Provide ongoing administrative and program support to ensure efficiency, compliance, and continuity across programs
  • Provide direct case management services to clients
  • Review and update program policies and procedures

Qualifications

  • Bachelor's degree in Human Services, Social Services, or a related field
  • Strong research, problem-solving, and writing skills
  • Excellent attention to detail
  • Dependable and punctual
  • Ability to manage multiple priorities in a fast-paced environment
  • Strong written and verbal communication skills
  • Experience supporting 245D programs, including IHS (formerly ILS), preferred
  • Experience working with case management systems such as SSIS and MnChoices 2.0 preferred
  • Experience with Star Services, Pavillio, and Reviving Care preferred
  • Background in human services or social services preferred

Job Types: Full-time, Part-time

Pay: $24.00 - $28.00 per hour
