Case Manager/Substance Abuse Counselor

Join our dynamic healthcare team as a Case Manager/Substance Abuse Counselor, where you will play a vital role in guiding individuals through their recovery journey and supporting their mental health needs. You will coordinate care plans, conduct comprehensive evaluations, and deliver evidence-based therapies in a compassionate, patient-centered environment.   • Conduct thorough intake assessments and patient assessments to determine individual treatment needs.
  • Develop personalized care plans utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy techniques.
  • Provide individual and group counseling sessions focusing on addiction recovery, mental health support, and trauma management, including PTSD care.
  • Implement crisis intervention strategies to manage acute behavioral health emergencies effectively and ensure patient safety.
  • Coordinate with healthcare providers, families, and community resources to facilitate comprehensive home and community care services.
  • Maintain accurate medical documentation, including progress notes, treatment plans, and compliance records in accordance with HIPAA regulations.
  • Proven experience in substance abuse counseling, behavioral health, or related clinical settings such as hospitals or community clinics.
  • Knowledge of mental health disorders such as PTSD, addiction medicine principles, and behavior management strategies.
  • Ability to perform patient care activities with empathy while maintaining professional boundaries; excellent case management skills are essential.

Minimum requirements for position are:

-Physical, TB test, Drug Screen, able to present in a professional fashion orally and physically, maintain and uphold strong boundaries while having a therapeutic manner.

-Current and valid RADT/SUDRC or similar credentials.

-If recovering from substance abuse, must have at least one year of continuous sobriety.

-Knowledge of 12 step base recovery a plus but not mandatory.

Pay: $25.00 - $30.00 per hour
