POSITION SUMMARY

The Case Worker - Crisis provides case management, assessment, and crisis intervention to individuals and their families in need of services related primarily to substance use or dual diagnoses such as mental health

QUALIFICATIONS

• Serves as member of a multi-disciplinary case management unit serving individuals with various needs; acts as a consultant to agency staff regarding mental health concerns; complies with Wisconsin State Statutes Chapters 34 and 51.
• Identifies full range of client needs through thorough investigation and assessment; differentiates between medical, psychological, vocational, social, familial, and daily living spheres.
• Designs individual service program for assigned cases; collaborates with program personnel in individual case management.
• Applies expertise in cognitive/behavioral therapies and/or other evidence based therapeutic interventions to determine appropriate treatment method; devises, implements, and monitors service plans for individuals; develops Relapse Prevention Plans, Crisis Plans, and Stabilization Plans for high-risk clients.
• Demonstrates motivational leadership to provide staff with support; works to achieve services and treatment objectives without duplication of effort and with maximum effective use of staff and resources.
• Directs knowledge of court processes to assist with court proceedings; consults with legal counsel on appropriate case specific matters; provides information for court petitions; attends court-ordered evaluations and hearings; provides honest and accurate testimony; monitors compliance with orders and Wisconsin State Statutes; explains consequences of non-compliance with court orders to clients who are involuntarily committed; monitors utilization of services and adherence to recovery plan; informs court of client’s adherence to court orders.
• Maintains awareness of community resources and support services available for individuals in crisis; educates clients about available community resources and how to independently gain access to needed services; secures, coordinates, and monitors necessary adjunctive services for individuals involving appropriate professional and paraprofessional staff in programs, through provider agencies, and other community resources.
• Transfers expertise of crisis intervention techniques, such as de-escalation strategies and active listening skills, to provide crisis intervention for individuals.
• Adheres to strict record-keeping and reporting guidelines and laws; documents client contacts and services; maintains client files and client specific data for reporting purposes; gathers information and completes state reporting requirements; prepares other records and reports, as necessary.
• Establishes and maintains effective working relationships with internal and external partners; serves as advocate for clients within the community; speaks to the community and other organizations to provide public education; attends and participates in staff meetings, supervision times, and other regularly scheduled meetings.
• Develops professional knowledge and skills by attending training events, conferences, and workshops.
• Performs other related duties as required or assigned.

TRAINING AND PROFESSIONAL EXPERIENCE QUALIFICATIONS

• Bachelor’s degree in social work or a related field.
• Six (6) months of experience in family, children, or adult case management services, or successful completion of an internship in the specific area of hire.
• Appointment will be conditional upon successful completion of criminal and caregiver background checks.
• Must have valid driver’s license, an acceptable driving record, and access to private transportation for work-related duties.
• Must possess and maintain personal automobile insurance in the amount of $100,000/$300,000.

DESIRED TRAINING AND PROFESSIONAL EXPERIENCE QUALIFICATIONS

• Experience in a County Department of Human Services handling adolescent services, adult services, case management, or service facilitation.
• Certified Social Worker or possess or ability to obtain a temporary certificate permitting the use of the title “social worker” with two (2) months of hire and become fully certified within the specified time limits of the temporary certificate and maintain certification thereafter during employment.
