Summary/Objective: The Case Worker manages the county office and provides comprehensive case management services to clients. Responsibilities include scheduling appointments, conducting interviews, assessing eligibility, developing service plans, facilitating workshops, making referrals, and maintaining accurate records. This role also involves engaging with community stakeholders to address client needs and support program goals.
Essential Functions:
1.Client Services:
o Schedule and conduct client appointments in Virtual ROMA to reflect appointments in ITFrontDesk, including interviews and assessments to determine eligibility.
o Develop client service plans with measurable goals and outcomes, providing case management and advocacy as needed.
o Monitor client progress to ensure compliance with program requirements and conduct follow-ups to track success.
o Explain program regulations, policies, and client rights and responsibilities during orientation workshops for new and returning participants.
o Analyze clients’ financial and eligibility information to determine initial or continuing eligibility for programs.
o Provide crisis intervention to assist individuals and families in achieving self-sufficiency.
2. Resource Coordination and Referrals:
o Mobilize county resources and partnerships to meet client needs.
o Serve as a liaison for inter-agency and outer-agency referrals.
o Contact energy vendors to prevent service disconnections for clients.
3. Workshops and Community Engagement:
o Develop and conduct client classes and workshops tailored to employment, education, or financial goals.
o Plan and implement special community awareness and education programs and projects as assigned.
o Conduct Public Hearings and organize Community Action Day annually.
4. Administrative and Reporting Duties:
o Prepare and submit monthly program reports by the last working day of the month.
o Submit Success Stories to the Case Manager Coordinator Quarterly.
o Maintain and secure accurate documentation, forms, and reports required for specific program areas.
o Enter and retrieve numerical and narrative data, and issue benefits through automated computer systems.
5. Community and Professional Relationships:
o Establish and maintain cooperative working relationships with program participants, department employees, regional employers, educational institutions, trainers, and other program stakeholders. Obtain MOUs with resources.
o Attend meetings, trainings, and workshops with other departments, government agencies, local organizations, and consortiums to support program service areas.
6. Other Responsibilities:
o Conduct home visits as needed.
o Perform peer review audits of files and Virtual ROMA.
o Prepare free income tax returns for clients.
Job Type: Part-time
Pay: $13.00 - $16.03 per hour
