Case Worker – Specialized Family Services Program

Job Summary
Join our dedicated team as a Case Worker in the Specialized Family Services Program, where you will play a vital role in supporting vulnerable children and families. We are seeking dedicated and compassionate Case Workers to support a federally funded    program focused on providing case management, safety, and well-being checks for vulnerable youth and young adults. This position plays a critical role in ensuring participants are safe, cared for, maintain engagement with required processes, and achieve positive outcomes through comprehensive case management. The ideal candidate possesses strong interpersonal skills, cultural sensitivity, sound judgment, and the ability to navigate complex situations while maintaining strict confidentiality and compliance with federal requirements.

Duties

  • - Conduct in-person site visits to confirm youth and young adult location and document findings.
  • - Perform follow-up site visits additional research as needed to resolve incomplete or conflicting case information.
  • - Assess youth/young adult living conditions and overall well-being, identifying and documenting indicators of abuse, neglect, abandonment or trafficking.
  • - Immediately report safety concerns to designated program authorities in accordance with required timelines and protocols.
  • - Collect, document (using Case Management system), and maintain accurate case data, including youth/young adult demographics, location status, household composition and school enrollment.
  • - Communicate with participants and sponsors to provide information on legal obligations, reporting requirements, and program expectations in a language appropriate manner.
  • - Coordinate with program leadership and stakeholders to provide status updates and support case resolution and closure

Requirements

  • Required Qualifications:
  • - High School diploma or equivalent.
  • - Experience working with vulnerable populations, such as child welfare, foster, adoption, or family preservation programs, juvenile justice or probation services, immigration services, case management or social services or related field.
  • - Collecting, documenting and maintaining accurate case records.
  • - Reporting or escalating safety concerns in accordance with established protocols and timelines.
  • Managing multiple cases or projects simultaneously, including follow-up actions and resolution of incomplete or conflicting information.
  • - Must be willing to travel to various locations across the US to complete verification and wellness process.

Desired Qualifications

While not required, the following credentials and experience is considered highly valuable to successful performance of this position.

- Bachelor's degree in Social Work, Criminal Justice, Sociology, Human Services or related fields.

- Spanish/English proficiency is a real plus

- Conducting field-based case work, including site visits, and conducting interviews.

- Assessing youth safety and living conditions for indicators of abuse, neglect, abandonment, or trafficking.

- Communicating requirements and expectations to diverse audiences both verbally and in writing.

- Handling sensitive and confidential information in accordance with established policies and standards of conduct.

Pay: $1,000.00 - $2,000.00 per week
