Description:

Astor Family Therapy & Support Services is a dynamic and supportive team looking for new members to provide educational advocacy and casework services to individuals and their families. Contacts in the home and the community are required, and agency vehicles are provided. When serving as an educational advocate this team member will work collaboratively with the school and family to address any barriers related to stabilization of the youth’s school status. Also, we want people who enjoy engaging youth in social activities to help build their self-esteem, learn new skills while providing guidance to community resources. Job Responsibilities:

Provides indicated direct casework services to client on assigned caseloads and in accordance with local department of social services regulations.

Responsible for submitting written reports for the client record as required by the agency according to time frames set by the program.

Responsible for preparing required external accountability reports and forms on case progress for assigned cases to local departments of social services, according to time frames set by local departments of social services.

Participates as a member of a treatment team for each assigned case. In addition, responsible for communicating with a multi-disciplinary team, including staff members, to ensure integrated service delivery to the child/family.

Along with Clinician/Therapist, serves as a primary provider of appropriate direct services to the child’s family based on need assignment and the treatment plan for the child/family.

Coordinates discharge planning and provides indicated aftercare service, including direct service and linkage to other supportive or needed services.
