Responsibilities

• Conduct thorough assessments of clients’ needs, including behavioral health, child welfare, and social service requirements
• Develop individualized care plans that incorporate crisis intervention, addiction counseling, and motivational interviewing techniques
• Provide immediate crisis management support during emergencies or urgent situations
• Coordinate with community resources such as Section 8 housing programs, Medicare services, and workforce development initiatives
• Offer mentoring and childcare support to foster stability and growth for vulnerable populations
• Supervise case management activities, ensuring compliance with policies and best practices
• Maintain detailed documentation of client interactions, progress notes, and service plans in accordance with organizational standards

Experience

• Proven experience in social work, behavioral health, or related fields involving crisis intervention and management
• Demonstrated success in addiction counseling or mental health support roles
• Supervising experience working with diverse populations is highly preferred
• Knowledge of child welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives is advantageous
• Strong skills in motivational interviewing, crisis management, and behavioral health strategies
• Experience providing childcare or mentoring services to at-risk youth or families is a plus
• Ability to handle sensitive situations with professionalism, transparency, and empathy
