Provides direct support and case management services to program participants, promoting a positive and spiritually uplifting environment aligned with the Grace-Based Model. This role ensures clients receive comprehensive assistance to achieve self-sufficiency and improved well-being.

RESPONSIBILITIES:

  • Foster a positive, grace-based atmosphere for clients and staff.
  • Interview applicants to determine program eligibility; conduct needs assessments and establish goals.
  • Develop intake and case plans; assist clients with required paperwork, orientation, and release forms.
  • Maintain accurate case records and logs; enter data into HMIS (Homeless Management Information System) and other required databases.
  • Aid in securing income, benefits, housing, and professional resources to enhance psychosocial well-being.
  • Meet regularly with clients to review progress, document updates and enforce program guidelines.
  • Provide tools such as proper hygiene care, budgeting, and scheduling, to ensure life skills are provided.
  • Prepare discharge plans and transition files for exiting participants.
  • Attend community meetings to promote the program and coordinate referrals.
  • Compile and submit monthly reports on services provided.
  • Monitor dorms and ensure clients are adhering to guidelines and rules.
  • Perform other related duties as assigned.

Education and Experience:

  • High school diploma or equivalent.
  • Minimum of five years’ experience working with individuals experiencing homelessness.
  • Proficient computer skills and strong organizational abilities.
  • Excellent communication and motivational skills, with the ability to build cooperative relationships with clients, colleagues, community partners, and volunteers.
  • Solid understanding of social service casework principles and community resources.
  • Ability to engage effectively with individuals from diverse cultural and racial backgrounds.
  • Exceptional customer service skills and flexibility to work varied schedules as needed.
  • Strong interpersonal and leadership skills; capable of working independently in a fast-paced environment.
  • Skilled in crisis management and de-escalation techniques.
  • Comprehensive knowledge of community resources.
  • Valid driver’s license with ability to travel; must maintain an acceptable annual Motor Vehicle Record check.

  • Driving: There is a requirement for this position to travel within downtown Jacksonville, and surrounding counties.

CRM Thrift Store serves the community by offering affordable, quality, merchandise as a Christian ministry to the general public. The revenue generated through the sale of donated items is used to support the ongoing work of the City Rescue Mission in providing food, shelter and services to the hungry, homeless and addicted in Northeast Florida.
