CBRF Registered Nurse
Transitions At Home is a trusted provider of Home Health/CBRF Services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes. We are seeking an experienced CBRF RN to support 3 CBRFs and play a vital role in coordinating and delivering patient-centered care.

What does a Transitions at Home CBRF RN do?
Transitions At Home CBRF RN will provide expert guidance and oversight in clinical and regulatory matters. This role includes staff training, critical thinking, care plan review, incident investigation, and assistance with audits and policy development. Ideal for an RN who has a strong knowledge of assisted living regulations. Who also coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Part-Time
- 20 -25 hours per week
- Monday through Friday
- No On-Call
- No weekends or Holidays

Qualifications / Job Requirements:

- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in CBRFs (preferred)
- CPR Certification
- Wound Care would be beneficial
- A valid Wisconsin Driver's License and Car Insurance
