CCM Case Manager

Job Purpose:
  • The CCM Case Manager is responsible for overseeing and coordinating chronic care management services. This role involves working closely with patients, healthcare providers, and other stakeholders to ensure comprehensive and continuous care for individuals with chronic conditions. The CCM Case Manager aims to improve patient outcomes through effective care planning, monitoring, and communication.

Key Responsibilities:
  • Develop and implement personalized care plans for patients with chronic conditions, ensuring alignment with their healthcare needs and goals.
  • Coordinate with healthcare providers, patients, and families to facilitate seamless communication and collaboration in the management of chronic care.
  • Monitor patient progress and adjust care plans as necessary to address changing health conditions and needs.
  • Educate patients and families about chronic conditions, treatment options, and self-care strategies to empower them in managing their health.
  • Ensure compliance with healthcare regulations and standards in chronic care management practices.
  • Maintain accurate and up-to-date patient records, documenting care plans, interventions, and outcomes.
  • Identify and address barriers to effective chronic care management, working to improve access to necessary resources and support.
  • Collaborate with multidisciplinary teams to enhance the quality and efficiency of chronic care management services.
Required:
  • Associates degree in Nursing
  • Nursing License in the state of Utah
  • Preference given to those with Nursing license in Nevada, Utah, Texas or combination of.

Required Experience:
  • Minimum of 2 years of experience in case management, preferably in chronic care management.
  • Experience working with diverse patient populations and coordinating care across multiple healthcare settings.
  • Proven track record of developing and implementing care plans for patients with chronic conditions.

Required Skills and Abilities:
  • Strong knowledge of chronic care management principles and practices.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient in using electronic health records and case management software.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Demonstrated problem-solving skills and the ability to make sound decisions in a fast-paced environment.
