Job Overview
We are seeking a proactive and detail-oriented CCM (Chronic Care Management) / RPM (Remote Patient Monitoring) Medical Assistant to join our dynamic healthcare team immediately. In this role, you will be instrumental in supporting patient care through efficient administrative tasks, accurate medical documentation, and seamless coordination of CCM and RPM services. Your energetic approach will help ensure patients receive timely, personalized care while maintaining compliance with healthcare regulations. Duties

  • Assist healthcare providers with managing CCM and RPM programs by scheduling appointments and coordinating patient follow-ups
  • Collect, verify, and document patient information accurately using electronic health records (EHR) systems
  • Support patients with enrollment and ongoing engagement in remote monitoring programs through clear communication and technical guidance
  • Prepare medical documentation, including progress notes, medication lists, and care plans with precision and attention to detail
  • Handle administrative support tasks such as answering patient inquiries, managing appointment calendars, and processing referrals
  • Ensure compliance with healthcare privacy laws by maintaining confidentiality of all patient information
  • Collaborate with the healthcare team to streamline workflows and improve overall patient experience

Experience

  • Prior experience working in a medical office environment is highly preferred
  • Knowledge of medical terminology relevant to chronic care management and remote patient monitoring is essential
  • Demonstrated ability to support medical administrative functions efficiently in a busy setting
  • Office experience that includes handling patient records, scheduling, and communication tasks is desirable
  • Experience in a dental or similar healthcare setting is a plus but not required
  • Strong organizational skills combined with excellent communication abilities are necessary for success in this role
