Solutions for Care (SFC) serves over 12,000 older adults and those living with disabilities each year in Berwyn, Cicero and Proviso Townships. We provide direct client service through information and assistance, in-home care, advocacy services and transitional assistance in hospitals. Primary Function:
Under the leadership of the Care Coordinator Supervisor, the CCP Support Staff is charged with intake and case aide duties. They are responsible for the initial screening of individuals, providing information and referrals to callers, and a variety of administrative tasks. They will assist the Care Coordinators by following up with clients regarding the submission of documents or any new developments with the clients’ case, in accordance with the State of Illinois Community Care Program. Additional responsibilities include coverage for receptionist, answering calls, and monitoring clients.

Education and other Requirements:

High School Diploma or GED is required.

Bilingual in English and Spanish is preferred.

One year of experience working with older adults or disabled individuals is preferred.
- A background check is required.

Proficient in Microsoft Office Suite (OUTLOOK, Word, Excel), Acrobat Reader ADOBE, ADP Payroll.

Strong communication skills, the ability to work with diverse populations, and proficiency with agency programs to maintain accurate reports are required.

Must be comfortable multi-tasking on the computer using different screens.

Criminal background check required.
Duties and Responsibilities:

Maintain current information regarding programs, benefits, and services available to seniors and adults with disabilities within our service area, including at the local, regional, state, and federal level.

Gather and accurately document relevant information from potential clients and their family members to make a first-level assessment of need and eligibility utilizing the IDOA Intake form.

Process DHS ABE Applications, client status changes, and terminations.

Display appropriate telephone etiquette during all phone call handling.

Navigate state databases such as Case Management Information Systems (CMIS), Participant Search Screen (PSS), and eCCPIS.

Receive case transfer requests and ensure they are processed according to timeframes.

Ensure that billing, coding, and case assignment timeframes are met for all referrals.

Generate proper billing records for data entry.

Meets regularly with supervisors to discuss priorities or other issues prior to case assignments.

Maintain accurate case note entries, documentation, reports, and record keeping.

Will cover for the receptionist as needed and will assist with other administrative duties as needed

Other duties as assigned by supervisor and senior leadership staff.

Must be able to sit and work at workstation for extended periods of time.

Must be able to walk, climb stairs, use elevators, and conduct visits in all kinds of weather, if requested by supervisor.
Schedule: Full time: 8:30 am to 4:30 pm, Monday thru Friday.
Salary:
Salary Range: $40,000 to $45,000. Performance reviews will be conducted every 12 months. Salary increases are based on performance.
