Position Summary
The Case Manager supports the delivery of person-centered services for individuals with intellectual and developmental disabilities enrolled in the organization’s Community Day Services (CDS) and Workshop (Production) programs. Working under the supervision of the Qualified Intellectual Disabilities Professional (QIDP), the Case Manager assists with coordinating supports, maintaining documentation, and ensuring each participant’s needs and goals are met.
This position serves as a vital link between individuals served, staff, and families—helping to ensure consistent communication, timely documentation, and regulatory compliance with standards established by the Illinois Department of Human Services (DHS), the Illinois Department of Public Health (IDPH), and the Centers for Medicare and Medicaid Services (CMS).
Essential Duties and Responsibilities
- Maintain an assigned caseload of participants and assist in coordinating daily services under the direction of the QIDP.
- Support the development, review, and implementation of Individual Service Plans (ISPs).
- Monitor participant progress toward goals and document outcomes accurately and timely.
- Assist the QIDP with completing required assessments, progress notes, and service documentation in accordance with DHS, IDPH, and CMS requirements.
- Maintain accurate records of attendance, service delivery, incidents, and progress reports.
- Work closely with Direct Support Professionals (DSPs), Job Coaches, and program staff to ensure services align with participant needs and ISPs.
- Assist with scheduling participant activities, appointments, and meetings.
Qualifications
- High school diploma or GED required; associate degree in Human Services or related field preferred.
- Minimum of 2 years of experience working with individuals with intellectual and developmental disabilities in a human services, vocational, or day program setting.
- Strong understanding of person-centered planning and documentation practices.
- Familiarity with DHS, IDPH, and CMS service and documentation standards preferred.
- Excellent communication, organizational, and interpersonal skills.
- Proficient in Microsoft Office and capable of learning electronic record systems.
- Ability to work effectively as part of a multidisciplinary team.

Pay: Up to $22.00 per hour
