Certified Behavioral Health Case Manager

CBHCM Certification FROM FLORIDA CERTIFICATION BOARD IS REQUIRED. We are seeking a compassionate and experienced Certified Behavioral Health Case Manager to join our healthcare team. This role involves coordinating and managing behavioral health services for diverse patient populations across inpatient, outpatient, and post-acute care settings. The ideal candidate will possess strong clinical skills, extensive knowledge of medical terminology, and experience in hospital and trauma center environments. The Case Manager will work closely with multidisciplinary teams to facilitate patient assessments, discharge planning, utilization management, and ensure compliance with HIPAA regulations. Responsibilities

  • Conduct comprehensive patient assessments to determine behavioral health needs and develop individualized care plans.
  • Coordinate discharge planning and post-acute care arrangements to ensure seamless transitions for patients.
  • Manage utilization review processes, including authorization of services and resource allocation.
  • Collaborate with medical teams, social workers, addiction counselors, and other healthcare professionals to optimize patient outcomes.
  • Maintain accurate medical records and documentation in compliance with HIPAA standards.
  • Facilitate diagnostic evaluations and case management interventions for patients across inpatient, outpatient, and clinic settings.
  • Assist in intake processes for new patients requiring behavioral health services.
  • Support patients through mental health counseling, addiction counseling, or other behavioral health interventions as needed.
  • Participate in multidisciplinary team meetings to review patient progress and adjust care plans accordingly.
  • Ensure adherence to hospital policies related to patient service, safety protocols, and regulatory requirements.

Skills

  • Proven experience in hospital environments such as Level I Trauma Centers or Level II Trauma Centers.
  • Knowledge of medical terminology, physiology, diagnostic evaluation, and discharge planning processes.
  • Experience with managed care, utilization management, and medical records documentation.
  • Familiarity with behavioral health practices including mental health counseling and addiction counseling.
  • Ability to perform comprehensive patient assessments and coordinate complex case management plans.
  • Excellent communication skills for effective collaboration with multidisciplinary teams and patients.
  • Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
  • Experience working in outpatient clinics or post-acute care environments is preferred.
