Certified Behavioral Health Case Manager (CBHCM)

Job Description
Renaissance Mental LLC is seeking a responsible, organized, and client‑focused Targeted Case Manager (TCM) to provide Medicaid Targeted Case Management services in Miami‑Dade and Broward County. This position requires strong documentation skills, service coordination, and effective communication with clients and community providers.

Responsibilities

• Provide Targeted Case Management services according to Medicaid standards
• Maintain accurate, complete, and timely documentation
• Coordinate community resources, medical services, and social supports
• Develop and update individualized service plans
• Conduct community visits and ongoing follow‑up with clients

Requirements

• Certified Behavioral Health Case Manager CBHCM (required)
• Experience in TCM or community services (preferred)
• Strong organizational and documentation skills
• Ability to work independently and professionally
• Bilingual (English/Spanish) is a strong plus — our agency speaks Spanish
