Duties

• Deliver comprehensive patient care by assisting with ADLs such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and ensuring patient comfort at all times
• Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, utilizing Electronic Health Record (EHR) systems to maintain precise medical documentation
• Observe and report any changes in patient condition promptly to healthcare professionals, ensuring timely interventions and optimal care outcomes
• Administer medications as prescribed in accordance with HIPAA regulations and medical protocols while maintaining strict confidentiality of patient records
• Support patients with specialized needs such as catheter care, Alzheimer's or dementia care, hospice support, and working with individuals with developmental disabilities or disabilities
• Assist with meal preparation and nutritional needs while maintaining cleanliness and safety standards within the home or facility environment
• Operate medical equipment such as Hoyer lifts and perform tasks related to hospice or memory care with compassion and professionalism

Requirements

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), recognized by state licensing boards
• Proven experience in home health care, nursing homes, assisted living facilities, or long-term care environments
• Strong knowledge of medical terminology and procedures related to patient care and safety standards
• Ability to perform patient observation diligently and recognize signs of distress or deterioration
• Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
• Experience working with individuals with disabilities or complex health conditions like Alzheimer's disease or dementia is highly valued
• Familiarity with EMR/EHR systems for accurate documentation of medical records and care activities
• Excellent communication skills to collaborate effectively with healthcare teams, patients, and families
• Ability to work independently in home settings while maintaining high standards of cleanliness—including laundry, cleaning, and meal preparation—ensuring a safe environment for all patients
