You will work across inpatient hospital settings, outpatient clinics, home health environments, and community care programs to provide comprehensive care plans tailored to each patient's unique needs. Duties

• Assess patients with lymphedema or related conditions, developing personalized treatment plans that incorporate manual lymphatic drainage, compression therapy, exercise, and trigger point therapy.
• Implement and monitor care plans for diverse populations including pediatrics, geriatrics, orthopedics, and patients in acute care or rehabilitation settings.
• Educate patients and caregivers on self-management techniques, discharge planning, and home-based exercises to promote ongoing health and wellness.
• Collaborate with multidisciplinary teams to coordinate care across inpatient units, outpatient clinics, home & community care programs, ensuring seamless patient transitions.
• Document patient progress accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
• Conduct discharge planning by preparing comprehensive reports that include physiotherapy assessments and recommendations for ongoing care at home or in community settings.
• Stay current with the latest advancements in lymphedema treatment modalities and integrate evidence-based practices into daily patient care.

Experience

• Proven experience working in inpatient hospital environments, outpatient clinics, or home health settings delivering patient-centered care.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy principles related to lymphedema management.
• Demonstrated ability to develop effective care plans for diverse populations such as pediatric patients or geriatric individuals.
• Familiarity with EMR systems for documentation and patient record management.
• Prior experience with discharge planning processes and coordinating care across different healthcare settings.
• Hands-on experience with trigger point therapy techniques and orthopedics is highly desirable.
• Excellent communication skills to educate patients about their condition and motivate them towards active participation in their recovery process.
