Join our dynamic healthcare team as a Certified Lymphedema Therapist (PT/OT), where your expertise will make a profound difference in patients' lives. In this vital role, you will deliver specialized therapy services focused on the management and treatment of lymphedema, utilizing your advanced knowledge of physiology, anatomy, and physiotherapy techniques. You will work across inpatient hospital settings, outpatient clinics, home health environments, and community care programs to provide comprehensive care plans tailored to each patient's unique needs. Duties
- Assess patients with lymphedema or related conditions, developing personalized treatment plans that incorporate manual lymphatic drainage, compression therapy, exercise, and trigger point therapy.
- Implement and monitor care plans for diverse populations including pediatrics, geriatrics, orthopedics, and patients in acute care or rehabilitation settings.
- Educate patients and caregivers on self-management techniques, discharge planning, and home-based exercises to promote ongoing health and wellness.
- Collaborate with multidisciplinary teams to coordinate care across inpatient units, outpatient clinics, home & community care programs, ensuring seamless patient transitions.
- Document patient progress accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
- Conduct discharge planning by preparing comprehensive reports that include physiotherapy assessments and recommendations for ongoing care at home or in community settings.
- Stay current with the latest advancements in lymphedema treatment modalities and integrate evidence-based practices into daily patient care.
Experience
- Proven experience working in inpatient hospital environments, outpatient clinics, or home health settings delivering patient-centered care.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy principles related to lymphedema management.
- Demonstrated ability to develop effective care plans for diverse populations such as pediatric patients or geriatric individuals.
- Familiarity with EMR systems for documentation and patient record management.
- Prior experience with discharge planning processes and coordinating care across different healthcare settings.
- Hands-on experience with trigger point therapy techniques and orthopedics is highly desirable.
