Job description

Adheres to the NEXUS HEALTHCARE MANAGEMENT Policies and Procedures and Compliance Program including all state and federal regulatory requirements. The Certified Medical Assistant is responsible for supporting the following: Remote Patient Device Program (“RPM”), Chronic Care Management (“CCM”) and Principal Care Management (“PCM”) for patients and Value Based Care teams for all NEXUS HEALTHCARE MANAGEMENT Clinics. The Certified Medical Assistant also ensures that all eligible patients are enrolled in all applicable programs by being a liaison between the corporate team and the clinic team.

JOB TYPE:

Full-Time and Part-time slots available.

Pay: $22.00 - $24.00 per hour

Expected hours: 40 per week

PRIMARY ACTIVITIES/DUTIES:

  • Certified Medical Assistant assists patients with managing their blood pressure and other chronic health conditions by daily monitoring and calls.
  • Certified Medical Assistant will have an assigned group of patients to follow up with monthly via phone.
  • Conversations include reviewing daily vitals, medical history and assisting patients to support the plan of care established by their provider.
  • Verify and record the patient’s demographics, medical issues, medications, and risk management factors to create a clinical summary using specialized medical software.
  • Contact patients when blood pressure exceeds established parameters to evaluate the situation.
  • Communicate with Practices and/or Providers, based on established protocols when reading exceed established parameters.
  • Help provide the patient with a means to make timely contact with healthcare practitioners in the practice who can address any urgent needs about their chronic medical condition.
  • Responsible for registering chronic care management and principal care management.
  • Comply with documentation requirements of the Medicare RPM, CCM and PCM programs by carrying out the care plan with the patient, family/caregiver and Providers as well as recording the data in the Electronic Medical Record (“EMR”)
  • Monitor adherence to care plans, evaluate effectiveness, monitor patients’ progress in a timely manner and facilitate changes as needed.
  • Provide patient health counseling, education, and instruction based on established protocols.
  • Handle potentially difficult conversations calmly and professionally.
  • Participate in individual & group projects as directed by management.
  • Participate in operational meetings for the NEXUS HEALTHCARE MANAGEMENT Care Coordination Program.
  • Audit care plans to ensure accuracy and compliance.
  • Participate in continuing education seminars.
  • Assist patients with software connection, including mobile application usage and enrollment procedures.
  • Ensure the program operates in compliance with applicable guidelines and regulations.
  • Assist in compliance monitoring to maintain and ensure program integrity.
  • Other duties, as assigned.

QUALIFICATIONS:

  • Ability to communicate with patients/customers at all levels via telephone.
  • Ability to follow HIPAA guidelines and regulations.
  • Must have technological competence for computer use.
  • Must be proficient with; MS Word, MS Excel, Email and Internet usage and Company EMR.
  • Must be able to sit for extended period of time
  • Experience in a call center environment is a plus but not mandatory.

PHYSICAL REQUIREMENTS:

  • Regularly uses a keyboard to interact with computer system.
  • Regularly sits for extended periods of time.
  • Regularly proofreads and/or reviews documents.

CUSTOMER SERVICE:

  • Responsible for supporting the values and customer service standards of NEXUS HEALTHCARE MANAGEMENT.
  • Accountable for outstanding customer service to all staff, patients, clients and vendors.
  • Develop and maintain effective relationships through effective and timely communication.
  • Initiate actions to respond, resolve and follow up regarding customer service issues with all customers and patients in a timely manner.

EDUCATION:

  • Certification as a Medical Assistant is required (CMA or Registered MA certification)
  • High school diploma or equivalent required.
  • Associate degree preferred.

WORK EXPERIENCE:

  • Minimum of 1–3 years’ work experience in healthcare setting involving patients with complex chronic disease preferred.

SKILLS:

  • Strong working knowledge of chronic diseases including chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, congestive heart failure and basic medical management.
  • Must be highly motivated, result-oriented with strong skills in presenting, communicating, organizing, multi-tasking and time management skills.
  • Strong organizational and interpersonal skills
  • Excellent customer service skills demonstrated by positive feedback from patients/team.
  • Ability to identify problems and recommend solutions.
  • Basic computer skills including previous work with an electronic health record (EMR)

  • Bilingual Spanish preferred
