Job Title: Remote Care Management Coordinator (1099 Contractor)

Medimote is seeking a compassionate and detail-oriented Care Management Coordinator to support Medicare patients enrolled in care management programs including RPM, RTM, CCM, PCM, BHI, and TCM.

This is a fully remote contract position supporting chronic care and transitional care services for medical practices.

Position Overview

The Care Management Coordinator will engage patients by phone and digital tools, support care plan activities, collect clinical and patient-reported information, and document all interactions accurately in accordance with Medicare care management requirements.

This role requires strong communication skills, excellent documentation, and the ability to work independently in a fast-paced remote environment.

Key Responsibilities

• Conduct scheduled patient outreach and follow-ups for Medicare care management services
• Collect and document patient-reported symptoms, vitals, adherence, and care plan updates
• Communicate with patients in a compassionate, professional, and patient-centered manner
• Support care coordination activities under clinical supervision
• Identify patient concerns and escalate appropriately to the clinical team
• Accurately document all encounters, assessments, and communications in the care platform
• Follow Medicare program requirements and internal workflows
• Assist with enrollment, engagement, and ongoing monitoring activities
• Support de-escalation and resolution of patient concerns when appropriate

Required Skills and Qualifications

• CMA (Certified Medical Assistant) certification preferred
• Experience in chronic care management, care coordination, or clinical support roles preferred
• Strong verbal and written communication skills
• Ability to speak with patients in a compassionate, empathetic, patient, and professional manner
• Strong documentation and clinical note-writing skills
• Ability to de-escalate difficult situations calmly and professionally
• High level of reliability, organization, and attention to detail
• Ability to learn new software and workflows quickly
• Comfortable working independently in a fully remote environment
