Job Summary
We are seeking a compassionate and dedicated Certified Nurse Assistant- Home Health Aide to join our dynamic healthcare team. Duties
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and other assistive devices
- Monitor vital signs and observe patient conditions, reporting any changes promptly to healthcare professionals
- Administer medications as directed under supervision, ensuring proper documentation in electronic health record (EHR) systems like Cerner or eClinicalWorks
- Support patients with complex needs including Alzheimer's care, dementia care, behavioral health challenges, and memory care
- Provide specialized care for individuals with developmental disabilities or those requiring hospice or palliative support
- Maintain strict adherence to infection control protocols using aseptic techniques and HIPAA regulations to protect patient confidentiality
- Document all patient interactions accurately within medical documentation systems, including vital signs, medication administration records, and progress notes
Requirements
- Valid certification as a Nurse Assistant (CNA) or Home Health Aide (HHA) with current licensure in good standing
- Experience working in diverse healthcare environments such as hospitals (including ICU, trauma centers, PICU), nursing homes, assisted living facilities, or outpatient clinics
- Demonstrated expertise in medical-surgical care, geriatrics, pediatrics, and acute or sub-acute care settings
- Proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate documentation and patient record management
- Knowledge of physiology and anatomy relevant to patient care needs across different age groups and conditions
- Ability to perform complex tasks including catheter care, phlebotomy (if certified), and use of medical equipment like Hoyer lifts or DME (Durable Medical Equipment) devices
- Strong communication skills to collaborate effectively with interdisciplinary teams and provide excellent patient service
- Physical ability to stand for extended periods, lift patients safely using appropriate techniques, and perform physically demanding tasks associated with caregiving
