In this vital role, you will provide assistance with daily living activities, ensure patient safety, and promote independence in a variety of settings including private homes, assisted living facilities, group homes, and nursing homes. This position is for a 6-hour split shift per day, typically consisting of 4 hours in the morning and 2 hours in the evening/bedtime routine, making it ideal for caregivers seeking flexible scheduling while providing consistent, quality care.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ vital signs and observe changes in health status to report to healthcare professionals promptly
• Support medication management by administering prescribed medications following HIPAA guidelines and care plans
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Operate mobility aids such as Hoyer lifts or transfer belts to ensure safe patient transfers and prevent injury
• Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and laundry
• Document patient progress, update care plans, and communicate effectively with healthcare teams to ensure continuity of care
• Assist clients with morning routines and evening bedtime care to ensure comfort and safety throughout the day

Qualifications

• Prior experience in home health care, assisted living, nursing home settings or group homes is highly preferred
• Knowledge of patient observation techniques, vital signs monitoring, and medication administration
• Certifications such as CPR, First Aid, HIPAA compliance training are advantageous
• Experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease or autism spectrum disorder is desirable
• Ability to lift heavy objects safely and perform physically demanding tasks as needed
• Strong interpersonal skills with a compassionate attitude towards clients’ needs
• Reliable transportation and flexibility to work split shifts, including mornings, evenings, or weekends as required

Pay: $18.00 - $20.00 per hour
