COMPENSATION: $25 per visit $5 Trip fee for inner San Antonio $10 trip fee outer San Antonio

Responsibilities

• Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote comfort and independence.
• Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems for seamless patient care coordination.
• Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care requirements, or behavioral health challenges.
• Use assistive devices like Hoyer lifts safely to prevent injury and facilitate patient mobility.
• Engage with patients in their home or facility settings, delivering compassionate care aligned with their individual care plans and physiological needs.

Experience

• Prior experience working in hospice or palliative care environments is highly preferred.
• Experience with EMR/EHR systems (Kinnser) to document patient information accurately.
• Familiarity with specialized care techniques such as catheter care, infection control practices for infectious disease care, and aseptic techniques.
• Knowledge of geriatrics, behavioral health issues, and working with individuals with disabilities enhances patient-centered support.

Pay: $30.00 per hour
