Duties
- Assist clients with daily living activities such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor and record vital signs, medication administration, and medical documentation accurately within EMR or EHR systems
- Provide specialized care for clients with dementia, Alzheimer’s, or memory care needs, ensuring a safe and supportive environment
- Support clients with complex health conditions including hospice, acute care, ICU, or behavioral health needs by following established protocols and aseptic techniques
- Collaborate with healthcare professionals to implement care plans that address individual physiology and anatomy considerations
- Ensure compliance with HIPAA regulations by maintaining confidentiality of medical records and patient information
- Engage in patient service activities that promote comfort and dignity while working with individuals with disabilities or developmental challenges
Skills
- Proven experience in long-term care settings such as nursing homes, assisted living facilities, or home care environments
- Familiarity with EMR/EHR systems like Cerner or eClinicalWorks for accurate medical documentation
- Strong knowledge of medical terminology, vital signs monitoring, catheter care, and basic life support procedures
- Experience working with diverse populations including pediatrics, geriatrics, or individuals with behavioral health needs
- Ability to perform phlebotomy and assist with DME (durable medical equipment) management when required
- Skilled in infection control practices including aseptic technique and handling infectious disease cases
- Excellent caregiving skills combined with compassion for patients facing dementia or memory impairments
