Complete documentation up to and including but not limited to: History & Physical assessment, Psych evaluation, medical progress notes, prescription documentation, any other documentation needed for patient care.
Daily detoxification visits and weekly residential visits with notes being completed within 24 hours.
Manage each clients case to be sure that all their needs are being met.
Early Discharge and direct transfer to another inpatient residential facility for continued treatment (successful or unsuccessful completion at another facility is irrelevant)
Relapse / Re-admits to DreamLife Recovery
AMA/AWOL/ADC interventions as needed or assigned
Counsel and advise individuals or groups with substance abuse problems as needed or assigned.
Develop and implement individual treatment plans.
Peer reviews as needed or assigned.
Establish treatment goals and milestones with clients.
Prepare progress reports and letters for relevant parties.
Provide routine nursing care supervision of clients in a professional manner.
Educate families and groups on substance abuse treatments and prevention programs.
Facilitate referrals to other healthcare or counseling professionals
Master's Degree required; Certified Registered Nurse License with State of PA or Physician Assistant License with State of PA.
Preferred two years experience in Mental Health.
All Required trainings and CEUs.
Pennsylvania license as an CRNP/PA
Current CPR Certification., BLS ACL, and AED Certification
Provides psychiatric care for patients, necessary to achieve DLR mission and objectives in collaboration with the Medical Director. And or another physician.
Reviews patients history as well as conducts consultations, assessments, and discharge interviews.
Counsels patients on all treatment options offered by the facility as indicated for their type of addiction and/or mental status.
Provides primary care that focuses on assessment, health management, education, advocacy and prevention.
Orders all necessary laboratory and radiological assessments; initiates consultations, referrals, psychotherapy and pharmacotherapy as indicated.
Responsible for inpatient clinical workload along with assigned administrative and professional responsibilities.
Coordinates patient care between nurses and counselors.
Develops and presents treatment plans to the multidisciplinary treatment team and participates in Treatment Team meetings.
Maintains effective and up-to-date documentation of progress notes.
Participates in required in service education (Bloodborne Pathogens, TB, Fire/Safety/Evacuation, Confidentiality.
Sexual Harassment, Workplace Violence, Compliance training,etc.) and facility-wide performance improvement (PI) activities and compliance activities aimed at preventing fraud, waste or abuse.
Supports the policies and procedures, rules and regulations of Dreamlife Recovery
Assists with the overall medical quality management of Dreamlife Recovery and/or sister facility providing leadership and guidance for clinical staff in accordance with standards of State and Federal regulations.
Functions as a liaison between patient/community.
Performs other duties and special projects as assigned.
Graduation from an accredited college or university with a major and coursework in nursing/physician assistant.
Licensed CRNP/PA in the State of PA maintains current license per state requirements.
Maintains current CPR certification.
Preferred 2 years' experience, with exposure to chemical dependency and/or dual diagnosed clients, preferably in an inpatient setting.
Previous experience in counseling, social work, or other related fields preferred.
Experience in conflict resolution.

Ability to build rapport with clients.
Excellent written and verbal communication skills.
