About the Role:

The LVN/RN plays a critical role in delivering high-quality nursing care within long-term care and skilled nursing facility settings. This position focuses on providing comprehensive direct patient care, including assessment, planning, implementation, and evaluation of nursing interventions tailored to individual patient needs. The RN serves as a patient advocate, ensuring that patients receive appropriate education about their health conditions and treatments to promote optimal outcomes. Additionally, the role involves specialized wound care management and collaboration with interdisciplinary teams to coordinate medical and surgical nursing care. Ultimately, the RN contributes to enhancing patient quality of life and safety through compassionate, evidence-based nursing practices.

Minimum Qualifications:

  • Current and valid LVN/RN license in the state of practice.
  • Completion of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of 1 year experience in long-term care or skilled nursing facility settings.
  • Strong knowledge of medical/surgical nursing principles and patient care standards.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Experience working with nurse practitioners and interdisciplinary healthcare teams.
  • Advanced certifications such as Certified Medical-Surgical Registered Nurse (CMSRN).
  • Experience in patient advocacy and education programs.

Responsibilities:

  • Provide direct patient care including monitoring vital signs, administering medications, and performing nursing procedures in accordance with established protocols.
  • Conduct thorough patient assessments and develop individualized care plans that address physical, emotional, and psychosocial needs.
  • Deliver patient education on disease management, medication adherence, and preventive health measures to empower patients and their families.
  • Manage wound care treatments, including assessment, dressing changes, and documentation to promote healing and prevent complications.
  • Advocate for patients’ rights and needs by coordinating with healthcare providers, families, and support services to ensure comprehensive care.
  • Maintain accurate and timely medical records and documentation in compliance with regulatory standards and facility policies.
  • Collaborate with interdisciplinary teams including physicians, nurse practitioners, therapists, and social workers to optimize patient outcomes.
  • Participate in quality improvement initiatives and stay current with best practices in nursing care and healthcare regulations.

Skills:

The required skills such as long-term care expertise and skilled nursing facility experience enable the RN to effectively manage complex patient needs in these specialized environments. Direct patient care and nursing care skills are applied daily to assess, monitor, and treat patients while ensuring safety and comfort. Patient education and advocacy skills are essential for empowering patients and families to participate actively in their care plans. Wound care proficiency is critical for managing chronic and acute wounds, reducing infection risks, and promoting healing. Collaboration with nurse practitioners and other healthcare professionals enhances coordinated care delivery and supports comprehensive treatment strategies.
