About the Role

We're looking for a skilled and compassionate Intensive Therapist (Intensive Care Coordinator) to provide wraparound care coordination for youth in the foster care system or at risk of out-of-home placement. You'll lead Child and Family Team (CFT) meetings, deliver direct counseling and case management, and coordinate across systems to help youth and families stabilize and thrive.

Caseload is manageable — 17 to 20 clients — so you can do this work well.

Community based position - meetings and remote work.

Key Responsibilities

• Deliver culturally sensitive, strengths-based individual, group, and family counseling and rehabilitation services
• Provide crisis intervention and stabilization for youth and families
• Coordinate care with multidisciplinary clinical teams and external agencies
• Facilitate CFT meetings and integrate findings into treatment plans and CANS assessments
• Complete documentation in compliance with Medi-Cal and Santa Clara County standards
• Facilitate referrals to community resources and perform targeted case management

Qualifications

• Master's degree in psychology, social work, or related field
• 2+ years working with high-needs children and families
• Experience with DSS, Mental Health, and county agencies preferred
• Valid California driver's license, current auto insurance, and clean MVR required (up to 50% travel)
• Bilingual in Spanish, Mandarin, or Vietnamese a plus — not required

Pay: $73,000.00 - $83,000.00 per year
