This position is a 0.5 FTE Children's Case Management
and 0.5 FTE MHP.
Job Summary
Fernbrook Family Center is seeking a compassionate and dedicated
Case Manager / Mental Health Practitioner (MHP)
to provide community-based mental health services to children, adults, and families.
In this role, you will support individuals who meet Minnesota State Statute criteria for:

Serious and Persistent Mental Illness (SPMI) (adults), and
-
Severe Emotional Disturbance (SED) (children)
You will help clients access essential services—including medical, social, educational, vocational, and financial supports—while also providing skills-based interventions to improve daily functioning. Services are delivered in home and community settings under the supervision of a Qualified Mental Health Professional (QMHP).

Key Responsibilities

Provide targeted mental health case management services to children, adults, and families

Deliver home and community-based mental health skills services
-
Maintain timely, accurate, and compliant documentation
-
Coordinate referrals and collaborate with community providers
-
Meet with clients regularly (minimum once every 30 days, or more as needed)

Participate in discharge planning and ensure smooth transitions from higher levels of care

Facilitate quarterly team meetings with clients and support systems
-
Advocate for clients to reduce barriers and resolve crises
-
Engage in supervision, staff meetings, and ongoing professional development
-
Complete 30 hours of mental health-related continuing education every two years

Meet productivity standards for both case management and clinical services

Education & Experience Requirements
Candidates must meet
one
of the following:
-
Bachelor’s degree in a behavioral science or related field (e.g., social work, psychology, nursing), OR
-
3–4 years of experience as a case manager associate, OR
-
Registered nurse with psychiatric training and 3 years of relevant experience, OR
-
4,000 supervised hours providing mental health services, OR
-
30 semester hours in behavioral sciences + 2,000 supervised hours
Additional Requirements

Valid driver’s license, reliable transportation, and auto insurance

Acceptable driving record

Ability to pass a DHS background check

Work Environment & Schedule
-
Flexible hours, typically between 7:00 a.m. – 7:00 p.m.

Regular travel to client homes, community locations, and Fernbrook offices
-
Full-time (40–43 hours/week) for salaried employees
-
Up to 40 hours/week for hourly employees (overtime requires approval)
Physical Demands

Prolonged sitting, observing, and computer use

Occasional lifting up to 15 pounds
Supervision
This position does not supervise staff but requires collaboration with interdisciplinary team members.
