La Puente Adult Day Health Care (ADHC) is a community-based program providing a variety of medical, rehabilitative, therapeutic, and recreational activities and services for older and/or disabled adults. We are looking for a qualified social worker to fill a few open positions at the center.

Qualifications

• Bachelor's in Social Work or Master Degree in Social Work/Gerontology/Psychology/Counseling
• Day Shift (Required)
• Mandarin (Required)
• Work authorization (Required)
• ADHC: 1 year (Preferred)
• Social Work: 1 year (Preferred)
• Case management: 1 year (Preferred)
• Leading Groups: 1 year (Preferred)
• Cantonese (Preferred)

Job Responsibilities

• Complete monthly counseling, reassessment assessment, quarterly notes, discharge plans, meeting notes, home visits and biopyschosocial assessments.
• Interact with center participants on a daily basis, assist with inquires, such as SSI, Calfresh, senior housing, Medical and Medicare benefits.
• Intake Assessment (Center tour, documentation, eligibility check and follow up as needed.)
• Complete assessments, progress notes, flow sheets, and quarterly reports in a timely manner.
• Other duties as assigned
• Must be bilingual in Chinese (Mandarin and/or Cantonese)

Education:

• Bachelor's (Required)
• Master's (Preferred)

Job Type: Full-time

Pay: $26.00 - $28.00 per hour
