Chronic Care Coordinator / Medical Assistant

Job Summary

Restore Health & Pain Treatment Group is seeking a compassionate, organized, and motivated Chronic Care Coordinator/Medical Assistant to join our growing team. This position combines traditional Medical Assistant responsibilities with Chronic Care Management (CCM) services to support patients with chronic health conditions and improve overall patient outcomes.

The ideal candidate will be a strong communicator who enjoys building relationships with patients, coordinating care, and working closely with providers in a fast-paced healthcare environment.

Essential Responsibilities

• Coordinate Chronic Care Management (CCM) services for eligible patients.
• Conduct monthly patient outreach and follow-up calls.
• Assist patients with care plans, referrals, and coordination of services.
• Educate patients on chronic disease management and treatment plans.
• Room patients, obtain vital signs, and update medical histories.
• Process patient triage messages.
• Schedule appointments, referrals, laboratory testing, and imaging services.
• Maintain accurate documentation in the electronic medical record (EMR).
• Support quality initiatives and patient care goals.

Qualifications

• Current Washington State Medical Assistant-Certified (MA-C) credential required.
• Previous Medical Assistant experience preferred.
• Experience with Chronic Care Management (CCM), care coordination, or patient outreach preferred.
• Strong communication, organizational, and multitasking skills.
• Experience with eClinicalWorks (eCW) or other EMR systems preferred.
• Ability to work independently and as part of a collaborative healthcare team.
• Willing to travel to Spokane as needed
