Chronic Care Management Medical Assistant

Description:

A part of our specialized team in delivering comprehensive, patient-centered care for individuals with complex chronic conditions. The ideal candidate will possess extensive clinical knowledge, work well with a team, and embodies excellence in patient care delivery. Conduct monthly outreach to enrolled patients of our in-house CCM program to assist patients in navigating the healthcare system and accessing necessary resources while monitoring patient progress and communicate any changes or concerns to the care team.

Full-time, with benefits.

Salary is based on experience.

Education:

High School Diploma or GED required.

Graduate of an approved Medical Assistant program.

Licenses/Certificates:

Completed Medical Assistant Program certificate

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
