POSITION SUMMARY

The Chronic Disease Nurse Supervisor oversees the Chronic Disease Case Management (CDCM) Nurses who provide chronic disease education, and case management support to both primary care patients and clients/patients living with HIV. The Chronic Disease Nurse Supervisor also provides support and oversight for planning, implementation, and evaluation of the coordinated Care Remote Care services. The position works with the clinical Director of Coordinated Care, Director and Assistant Director of Remote Care management, Chief Nursing Officer and is under the general supervision of the Chief Medical Officer and/or Primary Care Providers. The Chronic Disease Nurse Supervisor will assist to enroll referred and participating patients in the Remove Care Services. Services are provided via phone, telehealth, secure messaging, or in person. The mode of delivery may also apply to working with HIV + clients/patients for case management services and support. The Chronic Disease Nurse Supervisor will assist in ensuring all required paperwork, screenings, and education remain current and are completed in a timely manner. The nurse will continue to provide frequent monitoring/evaluation, documentation, follow-up, and collaboration to patient’s care team. This position demands the ability to interact with Federal, State, and local agencies to maximize the resources and to achieve program development goals.


Essential Functions Statement(s)

  • Supports the operation and progression of designated Coordinated Care Remote services.
  • Supports the selection and ongoing review of platforms, vendors, equipment, and educational materials to ensure department staff are meeting the needs of the target population.
  • Assists with developing and directing the implementation of annual program evaluation plan and performance improvement activities.
  • Participates in the center’s quality improvement activities and performs duties in accordance with applicable standards.
  • Assists in tracking the success of quality initiatives.
  • Guides and directs the activities of the Chronic Disease Case Management Nurses and other appropriate members of the Coordinated Care team; is the manager/leader of the Chronic Disease Care Management Nurses in assigned clinics and is responsible for performance evaluations, discipline as needed, supervision of direct care services provided by staff.
  • Collaborate with nurses and providers to determine process changes aimed at streamlining productivity.
  • Provides education/in-services to nursing staff as appropriate.
  • Ensures services are provided in an individualized and economical manner.
  • Develops and maintains relationships and partnerships with community groups, payors, and potential referral sources.
  • Coordinates and supports the efforts of the Remote Care Management Team in providing total patient care.
  • Communicates with staff through verbal or written reports to keep staff current of patient progress/needs/concerns.
  • Assist with policy and procedure revisions related to the programs.
  • Represent Arcare as a member of special committees when appropriate.
  • Perform all job duties in compliance with Arcare Safety and Infection Control Policies.
  • Assists in training staff regarding their participation in quality improvement activities of the organization
  • Assists in special projects as directed by the Coordinated Care Leadership staff
  • Promotes customer-driven excellence as a driver of organizational services.
  • Provide patient-centered care that incorporates culturally and linguistically appropriate services.
  • Educate patients about health maintenance activities, adoption of healthy behaviors, and support services available in the community.
  • Communicate with patients and families about health care issues and the importance of developing self- management goals and healthy behaviors.
  • Assist in coordinating seamless transitions of patient care across multiple care settings by supporting combined expertise of care team members.
  • Employ active listening techniques during interactions with patients, families, and team members.
  • Utilize quality data to identify process changes that will improve individual patient and population management.
  • Use health information systems efficiently to manage and improve care at the individual and system levels and ensure the delivery of timely and accurate information.
  • Incorporate quality indicators for specific disease states and /pr patient variables into patient care plans.

SKILLS & ABILITIES

Education: Registered Nurse


Experience: At least two years of experience in nursing, management experience preferred


Computer Skills: Microsoft Word and Excel; and customized software. Ability to learn and adapt to changing technologies as the organization changes. Ability to learn and utilize electronic record keeping systems.


Certificates & Licenses: Current RN license in appropriate state required. CPR and TB required at hire date and annually thereafter.


Other Requirements: The ability to relate with effectiveness to the public. The ability to effectively perform several duties of the department and the ability to organize and achieve goals in a busy setting as well as effectively communicate with internal and external stakeholders. Willingness to attend continuing education related to this role.
