Client Services Case Manager

Job Type: Part-Time

Pay: Starting at $17.00 per hour (Based on Experience)

NuLook Home Care Agency is seeking a compassionate, organized, and dependable Client Services Case Manager to join our growing team.

This position is ideal for someone who enjoys helping others, building relationships, solving problems, and ensuring clients receive the support they need to remain safe and independent in their homes and communities.

As a Client Services Case Manager, you will serve as a liaison between clients, families, caregivers, nurses, referral sources, and agency leadership. You will play an important role in supporting client satisfaction, monitoring service quality, coordinating follow-up activities, and advocating for the individuals we serve.

What You'll Do

  • Welcome new clients and families to our agency
  • Conduct client satisfaction and wellness calls
  • Follow up with clients regarding services and concerns
  • Track client hospitalizations and service interruptions
  • Assist with resolving client complaints and service issues
  • Monitor authorization renewals and reassessment needs
  • Coordinate communication between clients, families, caregivers, nurses, and office staff
  • Maintain accurate documentation and communication records
  • Participate in client visits and community outreach activities as assigned
  • Support special projects, client appreciation activities, and community resource initiatives

QualificationsRequired

  • High School Diploma or GED
  • Valid Driver's License
  • Reliable Transportation
  • Strong Communication Skills
  • Basic Computer Skills
  • Ability to Maintain Confidentiality

Preferred

  • Experience in Home Care
  • Experience in Healthcare or Human Services
  • Experience in Customer Service
  • Experience in Case Management or Care Coordination
  • Knowledge of Medicaid Programs
  • Experience Working with Seniors or Individuals with Disabilities

Schedule

  • Part-Time Position
  • Monday – Friday
  • Flexible Hours Based on Agency Needs
  • Occasional Local Travel for Client Visits

Compensation & Growth Opportunity

Starting Pay:
$17.00 per hour (Based on Experience)

Employees are eligible for a formal performance review after 90 days of employment. Team members who demonstrate strong performance, reliability, professionalism, and a commitment to our clients may be considered for a merit-based pay increase.
