Overall Summary:

With the collaboration of healthcare providers and clinic support staff, Clinic Nurse are responsible to provide quality, effectiveness, and efficiency in the deliverance and coordination of patient care, in order to facilitate patient satisfaction, clinic efficiency, and provider productivity. Clinic Nurse is responsible to assess, plan, implement, and evaluate patient care. Works in a collaborative and cooperative relationship with other members of the care team and commits to the organization’s mission to provide high quality comprehensive affordable healthcare to our communities.

Essential Responsibilities:

Under the general direction of the Clinic Nurse Supervisor, the Clinic Nurse performs a variety of complex, professional, analytical, and confidential medical services. Duties include:

  • Collaborate effectively with patients, families, care team, and community resources to provide holistic care.
  • Treat and educate patients; their family and the public regarding various medical conditions
  • Perform diagnostic tests and analyze results; operate lab equipment; administer medication
  • Perform care coordination and case management
  • Record medical histories and symptoms
  • Participate actively in the team-based care model
  • Administers medications, immunizations, performs medication reconciliation. Titrate medications/manages refills per clinic guidelines
  • Clinic skills include but are not limited to: vital signs, provides wound care, point of care testing, administration of immunization, and phlebotomy
  • Observes patient, records significant conditions and reactions, and notifies supervisor or provider of patient’s condition and reaction to drugs, treatment, and significant incidents
  • Initiates a patient education plan according to the individualized needs of the patient, as prescribed by provider, including patient and family instructions
  • Complete patient assessments and triage, record patient histories, symptoms and observations.
  • Ability to empathize with vulnerable populations and underserved patients
  • Assist in the coordination of patient care as a member of the health care team within the organizational framework of the healthcare setting and standards of practice.
  • Use safe methods of handling equipment and follows safety rules at all times.
  • Effectively utilizes population health methodologies such as patient registry, inreach, outreach, care protocols and team-based care to improve patient and population outcomes.
  • Organizes, supervises, makes adjustments and accepts responsibility for the quality of care provided patients and families by co-workers for a give work period; responsibility as a team leader when assigned.
  • Participate in performance improvement/quality control activities
  • Participate in the implementation of patient care standards, infection control standards and quality assurance criteria
  • Provides patient-centered care that is respectful of, and responsive to, individual patient preferences, needs and values. Provides and promotes excellence in customer service for both internal and external customers.

Secondary Responsibilities:

  • Assume general administration duties including fax, filing, copying, etc.
  • Attend webinars, workshops, training and/or meetings as requested by management.
  • Perform special project research and data collection activities as requested by management.
  • Analyze and interpret data, and prepare reports.
  • Coordinate, prepare, and distribute promotional flyers for events.
  • Plan and conduct trainings for both individual and group settings.
  • Perform additional clinic duties as necessary.
  • Rotate to other clinics or organization events, as needed.
  • Work weekends and evenings on a rotating basis, or as scheduled.
  • Other duties as assigned.
Requirements:

Qualifications:

  • A commitment to the organization’s mission to provide high quality comprehensive affordable healthcare to our communities.
  • Competent knowledge of database software, internet software, inventory software, order processing system, spreadsheet software and Microsoft Office Suite.
  • Must meet and maintain certificate and licensure requirement.

Certificates, Licenses and Education:

  • Nursing degree from an accredited college from an approved LVN/RN program.
  • California Registered Nurse or Licensed Vocational Nurse License
  • CPR/BLS Certification required

Physical Demands:

This position requires:

  • Sitting, walking, and standing associated with a normal medical, clinical and administrative office environment, with some bending, reaching, stretching, and lifting up to 25 pounds.
  • Manual dexterity for tasks involving standard office equipment and computer keyboard, including repetitive movements, such as typing.
  • Traveling to work sites, including some exposure to outside elements.
  • Exposure to potentially dangerous materials and situations, requiring adherence to extensive safety precautions and possibly the use of protective equipment.
